Abstract
Recently, leptospirosis became prevalent in Korea, and it was found to accompany
various hematological derangements.
We reviewed the hematological findings of 3 patients of leptospirosis who developed
anemia.
The results obtained are as follows:
1) The hemoglobin level was between 9.5 and 9.7%, reticulcyte counts were not
increased, erythroid cells of the bone marrow decreased and serum iron slightly
increased. Fragmented red cells were found and MCV increased in one case. Clinical
history and above findings suggested that the causes of anemia might be one of or
combination of hemorrhage, tone marrow suppression and microangopathic hemolysis.
2) White cell counts were normal or increased and shift to the left was found. Texic
granules, atypical Iymphocytes, plasmacytoid Iymphocytes and increase of the bone
marrow plasma cells were also found.
3) Thrombocytopenia was found and the f:efakaryocytes of the bone marrow were
normal or increased in number. B.T., P.T. and aP. T.T and fibrinogen level were
normal, which was not compatible with acute disseminated intravascular coagulation.
4) Hematological findings of leptospirosis were similar to those of Korean homorrhagic
fever in that increased myeloid series, decreased erythroid series, increased plasma cells
and appearance of plasmacytoid Iymphocytes in bone marrow and decreased platelet
counts were found.
Similarity was also found in so called hemorrhagic pneumonia-like disease in Korea in
that anemia, shift to the left of leukocytes, thrombocytopenia and increased M:E ratio
were noted.
